JDWNRH is the only national referral hospital catering tertiary medical care to the people of 20 Dzongkhags and technical backstopping to all the hospitals and Health centers. This hospital not only provides tertiary care but also provide feed back to those health facilities that have referred the patients to JDWNRH for their necessary follow up and management of the patient.
The medical referral committee of this hospital also refers many patients requiring special investigation, treatment, surgery, to India and third country. Therefore, maintaining up to date individualized patient record for reference is of utmost importance for each and every patient who attend this hospital as out patient (OPD) or get admitted in the ward.
Birth of Medical Record:
Patient record was maintained manually in the Out Patient Department and Ward by the health staff long before 1995. But, in 1995 on invitation of the Health Department Mr. Peter Parslow, who was the Administrative Officer of Mongar Leprosy Mission hospital introduced computerized patient record keeping system for the In-patients at JDWNRH. He developed the software in FoxPro and till December 2004 same software was being used. But since 1995 there have been vast changes in the medical services, health information system, and the use of computer software for medical data management system. The data we have today in our computer do not give adequate information. We are in the process of upgrading the software to accommodate new changes. But this requires logistic as well as Technical support, which PPD, in the Ministry of Health is extending all possible help.
Future plan:
There is a vision of fully computerizing the medical record keeping system through local area network (LAN). All patients attending JDWNRH will be registered using computerized system in the Registration counter and disease & treatment recorded in the Pharmacy unit. This type of computerized Individualized Patient Record (IPR) system will show a lifetime record of hospital visit with all the information of illness that one has visited for. Gradually all the wards, Doctor’s chambers, OPD chambers will be fully connected by LAN and all sorts of information can be accessed from the authorized computers.
To maintain accurate individual record of the person we collect vital information like Name, Date of birth, National I.D number of the patient or I.D of parents in case of minor, Father’s Name, Permanent Dzongkhag where one’s census is recorded, present Dzongkhag working or living at, Contact address in case of emergency. These are some of the important information one has to provide at the Registration Desk for the initial registration, but for subsequent visit it will be very easy to retrieve the information just by entering the Patient Registration Number given by the computer.
